Setup Procedure

This section describes the steps you need to follow to setup Symptom Survey Maestro for your 30 day trial extension.  After this initial setup you will be able to enter sample patients and surveys, run the analysis and print reports, for 30 additional days.  Please follow the steps below to setup Symptom Survey Maestro:

1.      Start Symptom Survey Maestro by double-clicking the icon on your desktop.

2.      Click on the Setup icon on the Task Bar.

3.      At this time you need to activate Symptom Survey Maestro by entering your registration information.  This information is located at the beginning of this document.  Click on the Activation Tab (bottom of screen).

a.       Click the Allow Edits switch to “ON”

b.      Enter your:
·         Serial Number
·         License Name
·         Activation Code

c.       It is very important to enter this information exactly as shown at the beginning of this document.

d.      Click on the Accept Registration button.  A message will be displayed that Symptom Survey Maestro is now activated for a new 30 day trial.

4.      Exit and then re-start Symptom Survey Maestro so that the registration information can be saved.
